Social Work Services
The Social Work Service at Herzliya Medical Center is included in the comprehensive care provided to inpatients and their families.
Social workers assist patients who face crises resulting from illness or hospitalization, as well as patients struggling to cope with this reality and its implications. They also provide assistance in preparing a patient for discharge from the hospital, ensuring the provision of information relevant to the full realization of patient rights, and coordinating all matters related to the Pregnancy Termination Committee.
The Social Work Service can be contacted via:
- Referral by the hospital staff
- A request made by the patient or a family member to a social worker or the treating team
Areas of activity of hospital social workers
- Coordination with the Pregnancy Termination Committee.
- Provision of information to inpatients and their families regarding rights and rehabilitation services provided through the HMO, the National Insurance Institute, and private nursing services in the community.
- At the request of the inpatient and in accordance with a physician’s recommendation, assistance in drafting a social report (with additional relevant medical documents attached) for referral to the patient's HMO for approval of a rehabilitation framework or a private recovery framework after hospitalization.
- Transfer of medical and social documents to the Holocaust Survivors Welfare Fund in order to obtain nursing assistance on completion of hospitalization.
- When necessary, and at the request of the inpatient, provision of short-term assistance coping with emotional difficulties caused by the illness and/or the hospitalization.
- Appointment of a guardian for inpatients for the purpose of additional surgical intervention, according to legal necessity.
